NJ Comprehensive Assessment Tool (NJCAT)

The New Jersey Comprehensive Assessment Tool (NJCAT) is the standard assessment DDD uses to gather and evaluate information about a person's need for support in three main areas: self-care, behavioral, and medical. The results also establish the person’s tier, which determines the annual budget they will have available for services.

Upcoming Changes, on September 24, 2026 DDD roled out an example of the update. You can click to see how they are going to be using tracked changes in the future here: NJCAT Refresh Tracked Changes - this refreshed NJCAT version is expected to go live in late 2027. It is not live currently.

In the interest of transparency, DDD is sharing the NJCAT Document with Tracked Changes to show the differences between the current NJCAT and the NJCAT Refresh. Please note: · The questions are not in sequential order because some were moved from a different section. The final version will use sequential numbering. DDD is anticipating releasing the NJCAT Refresh in Late 2027.

This coincides with person-centered planning when it comes to creating plans for individuals through DDD and the NJISP.

What is Person-Centered Planning?

Person-centered planning works for people of all ages and abilities. It is a collaborative approach to helping a person create a roadmap for their life that is based on their own strengths, preferences, and needs. 

Person-centered planning focuses on:

  • What’s important to the person (preferences and goals, people and things they enjoy)
  • What’s important for the person (health, safety, support)
  • The person's short- and long-term goals
  • The supports, services, and steps needed to reach those goals
