The Supports Program and Community Care Program were developed by the New Jersey Department of Human Services’ Division of Developmental Disabilities (DDD), which provides public funding for certain services that assist eligible New Jersey adults with intellectual and developmental disabilities, age 21 and older, to live as independently as possible. DDD Provider Search Information
Using DDD's web-based Provider Search Database, you can locate and learn about Support Coordination Agencies that serve the county where a person lives.
The Provider Search allows users to search for providers. You may filter search results by name, service, location (county, zip code), language, acuity, and residential licensure setting or use a combination of searches and filters. Click on the link above to view Services in your area.
Changing Your Support Coordination Agency
Once a person is enrolled in one of DDD's medicaid waiver programs, they have the right to change their Support Coordination Agency if they wish to. To request this change, submit the Support Coordination Agency Change Form to DDD.
