- Independent Discussion Tool, which is found in the documents below to download is to be used by the Support Coordinator and planning team any time an individual is interested in moving into an unlicensed setting without a natural caregiver present. This ensures the individual’s need for personal guidance and self-preservation skills has been discussed and a safe and supportive plan can be put into place prior to a move.
- Voluntary discharge of DDD services form can be found below in the documents section to download and is used when an individual/guardian requests discharge from the Division of Developmental Disabilities (DDD). Those who wish to dis-enroll from waiver services only (for example, they are not using a Waiver service at this time but have a BGS guardian) should instead use the Waiver Dis-enrollment Request Form.

FY2024 Wage Increase for Direct Support Professionals and Supervisors
The Division of Developmental Disabilities (DDD) is releasing the following information related to the January 1, 2024 Direct Support Professional (DSP) and DSP Supervisor wage increase. DDD is pleased to report that, due to the Fiscal Year 2024 (FY24) Appropriations Act, the January 1, 2024 wage increase will be about $0.50 higher (about $1.75 per hour as opposed to about $1.25 per hour).
